Your palliative care team will include a:
- Board-certified palliative care physician
- Nurse practitioner who is an expert in pain control and symptom management
- Nurse who assesses your condition and monitors symptoms and changes
- Medical social worker who provides grief counseling and emotional support for you and your loved ones
Illnesses that are appropriate for palliative care include: metastatic cancer, heart disease, lung disease, kidney disease, Alzheimer’s, multiple sclerosis, stroke, Parkinson’s and more. Patients are required to have a primary care physician and follow an established plan of care which may include grief counseling.
